POSITION SUMMARY:

A clinical nurse and management professional responsible for developing and growing the agency’s hospice programs, ensuring compliance with federal, state, and local guidelines for the hospice service line, and ensuring the quality of the agency’s delivery of hospice services to patients and families.  Please list your nursing license number with your resume. MAIN DUTIES:

  • Develops hospice programs, policies, and procedures
  • Identifies strategic partnerships, branding, and other mechanisms, for establishing PHR’s reputation as an outstanding provider of hospice services in the branches where hospice services are provided
  • Communicates and trains agency branch administrators and hospice staff on PHR hospice policies, procedures, and systems
  • Manage, and increase contributions to, the PHR Hospice Foundation, established to provide services for hospice patients and their families not covered by insurance payers
  • Trains and coaches branch administrators on hospice reporting, expectations, culture, and key success factors and branch sales staff on effective selling of hospice services
  • Plays a key role in recruiting, interviewing, selecting, and retaining hospice clinical managers and other specialty hospice disciplines (e.g., chaplains, bereavement services providers, volunteer coordinators)
  • Ensures that PHR gains Community Health Accreditation Program (CHAP) accreditation for its hospice services
  • Provides community education on PHR’s hospice services at key referral source locations
  • Completes regular reporting activities, assists PHR management and COO in making optimal operational and strategic decisions
  • Maintains weekly and monthly cognizance of key hospice performance metrics
  • Generates ad hoc reports and performs analyses as directed by the company management
  • Assists in establishing, promoting, and maintaining professional and cooperative business relationships with governing agencies, local businesses, the medical community, and educational venues
  • Maintains administrative records as necessary
