Professional Healthcare Resources, a leader in Home Health, Hospice and Personal Care services seeks a passionate and enthusiastic Patient Scheduler Coordinator to join our team in Lanham, MD. 

Summary and Responsibilities

Selects, trains, supervises, develops and disciplines PC aides. Ensures aides are assigned to patients based on needs and abilities and a good match. Coordinates the schedules of staff in accordance with company policies and payer authorizations. Makes home visits for supervision or training, and to ensure compliance with schedules and ensure customer satisfaction. In a branch with a large patient census, these duties may be separated

  • Interviews, selects orients and trains personal care aide staff. Ensures proficiency in RN skills assessment before first patient care assignment.
  • Accepts new referrals from Admissions and responds about staffing ability within 2 hours. Identifies the best aide to assign to each patient based on client needs, and aide skills and attributes.
  • Makes staffing assignments utilizing a priority staffing system to ensure that the staffing needs of all patients are met, adjusting work schedules to meet coverage needs.
  • Assigns and coordinates RN admission assessment visit and development of the aide care plan. Communicates the aide care plan to the aide in advance of assignment.
  • Ensures RN supervisory visits scheduled and completed per law and regulation, and agency policy.
  • Ensures original time sheets are received per policy, are accurate, and complete, and that all time sheets for the period are obtained and processed, to have a smooth payroll process.

Education and/or Experience
  • Must possess thorough knowledge of home health practices, procedures, and professional standards of care as set by Federal and state regulations, as well as Medicare regulations for home health services.
  • Three (3) years of home health care experience preferred
  • Previous experience of health agency intake processes preferred
  • Proficient in reading and writing in English
