Full Time, Part Time , PRN position available

POSITION SUMMARY:

A clinical professional responsible for insuring quality total patient care through assessment, planning, interventions, and evaluation of patient needs through nursing care plans for each assigned patient. Under the direction of the physician, will address patient and family needs; use evidence-based practice; directs Licensed Practical Nurses, Certified Nursing Assistants, and Home Health Aides in quality patient care; provides appropriate patient/family/caregiver teaching; promotes quality service in accordance with established agency policies and procedures; insures compliance with federal, state, and local guidelines for all services; meets budgeted productivity standards; assures the quality and growth of the agency and; participates in the education of facility and community healthcare systems

 

  • Performs initial and ongoing comprehensive assessments
  • Acts as case manager by coordinating all patient/family services and prioritizing the patients needs with the members of the interdisciplinary team
  • Applies specific criteria for admission and re-certification to hospice care, as applicable, to establish appropriate levels of care and patient's eligibility
  • Initiates appropriate palliative interventions/symptom management and monitors effectiveness; provides referrals to other services and disciplines as needed
  • Accurately and completes all clinical documentation per agency policy
  • Performs accurate medication review and provides education as needed
  • Identifies and communicates changes in the patient's condition; informs the physician and obtains new orders as needed
  • Coordinates the discharge from service if appropriate or bereavement follow-up after hospice death

The successful candidate must have a Minimum of (2) two years of Licensed Practical nursing or Registered nursing experience preferred, with the skills below:

  • Compassion: Hospice nurses help people at the end of their lives. They need to be sympathetic to a patient's needs, and be able to deal with people in various states of pain, trauma and tragedy.
  • Attention to Detail: Hospice nurses can help doctors administer medications, monitor vital signs and subtle changes in a patient's health. Attention to detail is crucial.
  • Organizational Skills: Hospice nurses will face patients with differing needs, stages of health and risks. Being organized and knowing how to prioritize will be helpful.
